Rates are per 45-minute session. Please contact me for details.

Insurance
I am an in-network provider for Empire Blue Cross Blue Shield, Northwell Direct and Aetna . If you do not have BCBS, Northwell Direct or Aetna, services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage for out-of-network providers carefully by asking the following questions:

  • Do I have outpatient mental health insurance benefits?
  • What is my deductible and has it been met?
  • Does my deductible apply for mental health services?
  • Do I have coverage for out-of-network providers?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?

Payment
Cash, check, credit card, debit card, HSA or FSA are accepted for payment. Please note session fees and co-pays not paid at the time of the appointment are subject to a $5.00 surcharge.

Cancellation Policy
If you do not show up for your scheduled therapy appointment or if you have not notified me at least 24 hours in advance of a cancellation, you will be required to pay a $50 fee.

You have the right to receive a “Good Faith Estimate” 

explaining how much your medical care will cost

  • Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. 
  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. 
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 
  • Make sure to save a copy or picture of your Good Faith Estimate.
